1.What we ask
- What you would most want made clear about your migraines
- How long migraine has been part of your life
- How many attacks you have per month, on average
- What usually happens with tracking when an attack hits
- Whether you experience aura symptoms, and which kinds — visual, sensory, speech and language, none, or another kind you can describe in your own words
- What would matter most to you if one thing became clearer
Every question has a "Skip this question" control. You do not have to answer any of them to continue.
One thing worth calling out: the aura question has a free-text option, so anything you type there is health information you have written yourself. It is handled exactly like the other answers — it stays in your browser — but please don't type anything you would not want to write down.
At the end we ask for your name and email address to create your account and carry your subscription into the app.
Under privacy laws your answers here are "consumer health data" (Washington My Health My Data Act and similar US state laws), "sensitive personal information" (California CPRA), and "special category data" (EU/UK GDPR). We collect and handle them under the heightened protections those laws require.
2.Your consent
Before the first question, on a screen that asks you for nothing else, you tick an unchecked box:
"I agree to Amaina collecting the health-related answers I give in this questionnaire, as described in the Health Questionnaire Privacy Notice."
The box is never pre-ticked, and the Begin button does nothing until you tick it. This is your opt-in consent (Washington My Health My Data Act and similar) and, for EU/UK visitors, your explicit consent under GDPR Article 9(2)(a). It is a separate act from accepting our Terms of Use and Privacy Policy.
Your name and email are a separate purpose — creating your account and sending you your download link — described in the Privacy Policy.
Declining. Skip any question you'd rather not answer, or close the page at any point. Nothing you entered is kept either way.
Withdraw any time by emailing legal@smart-it.io. Withdrawing doesn't affect anything we lawfully did before you withdrew.
3.What happens to your answers — the short version is nothing
Your answers are held in the page itself, in your own browser, while you answer. They are not sent to our servers, not sent to our funnel provider, not written to our CRM, not attached to your payment, and not sent to any advertising platform. They are discarded when you close the tab or finish the questionnaire.
Two consequences, and we'd rather state them plainly than let you assume the friendlier one:
- We cannot show you, correct or delete your answers on request — not because we decline to, but because we never had them.
- Nothing you answered is carried into the Amaina app. The screen that says your summary is ready is telling you that you finished the questionnaire, not that a profile was saved. When you sign in to the app with the same email, your subscription is there; the app will ask you its own onboarding questions.

None of the above carries your health answers. Advertising and analytics receive page and conversion events only — never what you answered. We do not build custom, lookalike or health-interest audiences, and we make no automated decision with a legal or similarly significant effect on you.
The price and the plans you are shown do not depend on your answers — they are the same for everyone who reaches the paywall.
5.How long we keep things
- Your questionnaire answers: we never receive them, so we keep nothing and there is nothing to delete.
- Your name and email: for as long as you have an account or are on our mailing list, and after that only as long as tax and accounting law requires for the payment record.
- Your subscription and payment record: as required by tax and accounting law.
- Aggregate analytics that can no longer be tied to you: no time limit.
Deletion requests are honored within 30 days unless a law requires us to keep the data longer.
6.Payment and subscription
This questionnaire ends with an offer of a paid, auto-renewing subscription, purchased on this website through Stripe — not through Apple:
- Price, billing period, renewal terms and the money-back guarantee are shown on the checkout page before you enter any payment details, and are set out in our Terms of Use.
- Your subscription renews automatically until you cancel.
- You can cancel at any time on your billing page — the link is in the subscription emails you receive and is published on amaina.health. Cancelling takes effect at the end of the period you have already paid for.
- We are the seller of record for web subscriptions, not Apple — Apple's "Report a Problem" route does not apply. Refund requests go to legal@smart-it.io.
7.Your rights
You can see, correct, or delete the personal data we hold about you, withdraw consent, limit how we use sensitive information (California), and opt out of any "sale" or "sharing" — we don't sell or share health answers, and on this funnel we do not even hold them. Email legal@smart-it.io; we respond within 30 days (45 for California). If your browser sends a Global Privacy Control (GPC) signal, we honor it as an opt-out of "sale"/"sharing". The questionnaire and the app are for adults 18 or older; you confirm your age at the sign-up step, and we don't knowingly collect health information from anyone under 18.
